Required Athletic Paperwork:

Students registering to participate in any sport offered by the Rhode Island Interscholastic League must complete online registration each season of participation and  read and sign the following forms:

1.) Athletic Consent Form: this form just needs to be filled out one time during your four years at The Prout School.

2.) Assumption of Risk Form: this form needs to be filled out by all athletes one time during their time at Prout. The Assumption of Risk must be notarized and a hard copy turned in. 

3) Current Physical: must be uploaded on the magnus site through the nurses' office  before the student is allowed to participate in practice/tryouts. To ensure clearance for participation, physicals must be dated and signed by an authorized physician and uploaded prior to the first day of practice/tryouts. Physicals are valid for 13 months from date of the last exam.
